Service
Mental Health Skill Building Services are services facilitated within the home which include intensive individual and family counseling, crisis intervention, medication management assistance, social skills development, conflict resolution, community resources, activities of daily living, functional skills improvement and case management.
A comprehensive assessment of the client is completed and an individual service plan (ISP) is established to address individual and family needs. We will provide close collaboration with other involved providers in an effort to optimize service..

Eligibility Criteria
Individuals qualifying for this service must demonstrate a clinical necessity for this service due to mental, behavioral or emotional illness. The following criteria must be met in order to receive services:
Have a need for individualized training in acquiring basic living skills.
Have a qualifying mental health diagnosis (Psychotic disorder, major depressive disorder-recurrent, or bipolar I or bipolar II).
The individual must have a prior history of qualifying mental treatment/hospitalization.
The individual has had a prescription for an antipsychotic, mood stabilizing, or antidepressant medication within the 12 months prior to the assessment date.
